This form can be used as a grading record/transcript for grades K-8 cumulative files. It is not required by ECS, but it is highly suggested that you keep a record of your student’s achievements from year to year. ECS will accept these forms and securely file them upon request.
Student Name – write the name of your student (first & last is fine, but complete name is more official)
Year – enter the completed school year (ex. 2009-2010)
Grade Level – enter the grade completed or use Primary for 1st-3rd and Secondary for 4th-6th
Starting Date – the first day of school completed for the past year
Ending Date – the last day of school
Days Present – number of school days completed
Days Absent – (this should really be zero since you have freedom to adjust when days are missed)
Total School Days – number of school days completed (should be the same as Days Present)
Height – list height of student from ending of the year (option on printable form)
Weight – list weight of student from ending of the year (option on printable form)
Comments – Be careful what information is listed here. Remember that this information may follow your child in their cumulative file.
Subject/Class – Common subjects have been entered, and you can write your own title next to them if needed. You can even cross out those that do not apply.
1st Semester – List grades for 1st half of the year if desired. You can also use this for older students that only study a subject for half a year.
2nd Semester – If you listed 1st Semester grades, you will want to list the grades for the second half of the year here.
Overall Grade – Using the Grading Key, assign a grade for each subject. It is suggested that you either use O,S,N or the letter grades and do not mix up the different codes. Also, normally grades K-3 use O,S, N, but the upper level grades use letter grades. You could also simply use Complete and Incomplete, though this may be hard for another school to translate. Also, you can give percentage grades with assigned letter grades too (ex. 93/A) If you listed semester grades, the overall grade should display an average of the two.
Promote to Grade Level – if there is promotion, list the next grade level here (option on printable form)
Official Signature – the parent/teacher’s signature is fine, or the ECS administrator can sign and verify it for you. If you submit this online, your email address will be used as your electronic signature.
